Using levodopa together with pramipexole may increase the effects of levodopa. Contact your doctor if you experience nausea, diarrhea, vomiting, weakness, fainting, confusion, hallucinations, muscle twitching, and agitation. If your doctor does prescribe these medications together, you may need a dose adjustment or special test to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

ADJUST DOSE: Pramipexole may potentiate the dopaminergic side effects of levodopa, resulting in development or exacerbation of dyskinesia. In clinical trials, other adverse effects such as hallucinations, orthostatic hypotension, extrapyramidal syndrome, insomnia, and dizziness were also increased in patients with advanced Parkinson's disease treated with the combination compared to levodopa alone. Pharmacokinetically, pramipexole has been reported to increase the serum peak levels (Cmax) of levodopa by 40% and decrease the time to reach peak levels (Tmax) from 2.5 to 0.5 hours without affecting the overall extent of absorption. However, a study consisting of 26 patients with Parkinson's disease stabilized on levodopa/carbidopa therapy found no significant changes in levodopa pharmacokinetics following the addition of pramipexole for 7 weeks.

MANAGEMENT: A reduction of levodopa dosage should be considered during coadministration with pramipexole. In a controlled study in advanced Parkinson's disease, the dosage of levodopa was reduced by an average of 27% from baseline.
