Using rifAMPin together with pravastatin can decrease the effects of pravastatin. Contact your doctor if your condition worsens or if your cholesterol increases. You may need a dose adjustment or special test to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: Coadministration with rifampin may decrease the oral bioavailability of pravastatin in some patients. The mechanism involves reduced absorption as well as increased first-pass biliary excretion of pravastatin due to rifampin induction of canalicular multispecific organic anion transporter (cMOAT or MRP2), an efflux transporter expressed in the kidney, liver, and intestine and of which pravastatin is a substrate. In 10 healthy nonsmoking volunteers, pretreatment with rifampin (600 mg orally once daily for 5 days) decreased the mean area under the plasma concentration-time curve (AUC) of pravastatin (40 mg single oral dose) by 31% compared to placebo. However, there were large interindividual differences in the effect of rifampin, as pravastatin AUC decreased in 7 subjects (more than 50% in 5 of them) but increased in 3 subjects when compared to placebo. Rifampin also decreased the peak plasma concentration (Cmax) of pravastatin on average by 21%, but the difference was not statistically significant.

MANAGEMENT: Although the effect of rifampin on pravastatin was relatively small compared to that reported for other HMG-CoA reductase inhibitors, the interaction may be clinically significant in some patients. Dosage adjustment for pravastatin may be necessary during long-term treatment with rifampin.
