Consumer:

Astemizole and terfenadine are no longer on the market in the US. Do not take astemizole or terfenadine with any other medication.

Professional:

CONTRAINDICATED: Coadministration of a single 430 mg dose of quinine increased plasma concentrations of astemizole and its metabolite, desmethylastemizole. The mechanism is inhibition of hepatic isoenzyme metabolism of astemizole. Elevated levels of astemizole and desmethylastemizole may cause a prolongation of the electrocardiographic QT interval and potentially fatal ventricular arrhythmias. One case of torsades de pointes has been reported in a patient with mild electrolyte disturbances. Quinine may also interact with terfenadine in the same manner. Terfenadine and astemizole are metabolized via the same CYP450 enzymatic pathway, and elevations in plasma terfenadine levels have produced similar cardiac events.

MANAGEMENT: The concomitant use of quinine and astemizole or terfenadine is considered contraindicated.
