Using quiNIDine together with dilTIAZem can increase the effects of quiNIDine and affect the rhythm of your heart. Contact your doctor if you experience ringing in the ears, hearing loss, visual disturbances diarrhea, dizziness, weakness, headache, difficulty breathing, and an irregular heartbeat. You may need a dose adjustment or special test if you use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Professional:

MONITOR: Diltiazem has been reported to increase the effects of quinidine. In healthy volunteers the area under the concentration-time curve of quinidine was increased by 51% and the half-life was prolonged from 6.3 to 9.3 hours. The mechanism is unknown, but may be related to inhibition of CYP450 3A4 hepatic metabolism. QT prolongation has been reported. However, data have been conflicting: another study reported no interaction between diltiazem and quinidine.

MANAGEMENT: Close monitoring of quinidine levels, pharmacological effects, and ECG, and QT interval is recommended if diltiazem is added to or withdrawn from therapy. Alternatively, felodipine and amlodipine do not inhibit CYP450 3A4 and are not expected to affect quinidine levels. Patients should be advised to notify their physician if they experience tinnitus, hearing loss, visual disturbances, diarrhea, dizziness, weakness, headache, syncope, or palpitations.
