Using warfarin together with mirtazapine may occasionally increase the risk of bleeding. Talk to your doctor if you have any questions or concerns. Your doctor may be able to prescribe alternatives that do not interact, or you may need a dose adjustment or more frequent monitoring of your INR to safely use both medications. You should seek immediate medical attention if you experience any unusual bleeding or bruising, or have other signs and symptoms of bleeding such as dizziness; lightheadedness; red or black, tarry stools; coughing up or vomiting fresh or dried blood that looks like coffee grounds; severe headache; and weakness. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: Limited data suggest that mirtazapine may potentiate the hypoprothrombinemic effects of warfarin. The mechanism of interaction has not been established. In 16 healthy male study subjects, mirtazapine 30 mg once daily at steady state caused a small but statistically significant increase (0.2) in the international normalized ratio (INR) during treatment with warfarin. The possibility of a more pronounced effect with higher dosages of mirtazapine cannot be excluded. In an observational case-control study using U.S. Medicaid claims data from 1999 to 2005 covering 430,455 warfarin users with a total of 407,370 person-years of warfarin exposure, investigators found that initiation of various antidepressants including mirtazapine was associated with an increased risk (approximately 1.5--fold odds) of serious gastrointestinal bleeding. By contrast, a retrospective analysis of the medical records of 6772 warfarin-treated inpatients from a Finnish hospital reported no increased risk of bleeding with concomitant use of non-SSRI antidepressants such as mirtazapine.

MANAGEMENT: The INR should be monitored more closely following initiation, discontinuation, or change of dosage of mirtazapine, and the warfarin dosage adjusted as necessary. The same precaution may be applicable during therapy with other vitamin K antagonists, although clinical data are lacking.
