Professional:ADJUST DOSE: The coadministration of rifampin and rofecoxib may result in decreased plasma concentrations of the latter. According to the manufacturer, rifampin 600 mg daily produced an approximately 50% decrease in plasma rofecoxib concentrations. The mechanism is accelerated clearance of rofecoxib due to enhanced CYP450 3A4 enzymatic activity by rifampin.
MANAGEMENT: A starting dosage of rofecoxib 25 mg once daily should be considered for the treatment of osteoarthritis in patients receiving rifampin therapy. Monitoring for altered efficacy and safety is recommended.
