Professional:CONTRAINDICATED: Coadministration with rifampin may significantly reduce the plasma concentrations of voriconazole. The mechanism is rifampin induction of CYP450 isoenzymes that are responsible for the metabolic clearance of voriconazole. According to the manufacturer, rifampin (600 mg once daily) decreased the steady-state peak plasma concentration (Cmax) and area under the concentration-time curve (AUC) of voriconazole (200 mg every 12 hours for 7 days) by an average of 93% and 96%, respectively, in healthy subjects. Doubling the dose of voriconazole does not restore adequate systemic exposure to voriconazole during coadministration with rifampin.
MANAGEMENT: The use of voriconazole with rifampin is considered contraindicated.
