RifAXIMin may alter the blood levels of warfarin. Both increased or decreased effects may occur. Talk to your doctor if you have any questions or concerns. Your doctor may be able to prescribe alternatives that do not interact, or you may need a dose adjustment or more frequent monitoring to safely use both medications. Contact your doctor if you experience any unusual bleeding or bruising, or have other signs and symptoms of bleeding such as dizziness; lightheadedness; red or black, tarry stools; coughing up or vomiting fresh or dried blood that looks like coffee grounds; severe headache; and weakness. Also be alert to potential signs and symptoms of blood clots such as chest pain, shortness of breath, sudden loss of vision, or pain, redness or swelling in an extremity, and seek immediate medical attention if they occur. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: Coadministration with rifaximin may increase or decrease the anticoagulant effects of warfarin. The mechanism for this interaction has not been clearly delineated. Bleeding events have been reported in patients maintained on warfarin and given rifaximin. It is unknown if other oral anticoagulants are similarly affected.

MANAGEMENT: Caution is advised when rifaximin is used concomitantly with warfarin, particularly in patients with hepatic dysfunction. The International Normalized Ratio (INR) or prothrombin time and clinical response should be monitored closely during concomitant therapy. Adjustments in warfarin dosage may be necessary whenever rifaximin is added to or withdrawn from therapy in order to maintain the desired level of anticoagulation. Patients should be advised to promptly report possible signs of bleeding (e.g., pain, swelling, headache, dizziness, weakness, nosebleeds, bleeding of gums from brushing, unusual bruising, red or brown urine, or red or black stools) or signs of blood clots (e.g., chest pain, shortness of breath, sudden loss of vision, or pain, redness or swelling in an extremity).
