Using regorafenib together with rosuvastatin can increase your blood levels of rosuvastatin and may cause a dangerous side effect on your muscles. If your doctor prescribes these medications together, you may need special tests to safely take both medications. You should also watch for muscle symptoms and call the doctor if you have muscle pain, tenderness, or weakness, especially if you also have dark colored urine. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: Coadministration with regorafenib may increase the plasma concentrations of drugs that are substrates of the breast cancer resistance protein (BCRP) transporter. The proposed mechanism is decreased clearance due to BCRP-inhibition by regorafenib and its active metabolites, M-2 and M-5. When regorafenib (160 mg for 14 days) was given prior to a single dose of rosuvastatin (5 mg), a BCRP substrate, rosuvastatin mean systemic exposure (AUC) and Cmax increased 3.8-fold and 4.6-fold, respectively.

MANAGEMENT: Caution is advised if regorafenib is used concomitantly with drugs that are substrates of the BCRP transporter. Monitoring for signs and symptoms of increased exposure to the BCRP substrate should be considered whenever regorafenib is added to or withdrawn from therapy.
