Consumer:

Talk to your doctor before using ramelteon together with doxepin. Combining these medications may significantly increase the blood levels and effects of ramelteon. This can lead to increased side effects such as excessive drowsiness, abnormal thinking, behavioral changes, and depression. You may need a dose adjustment or more frequent monitoring by your doctor to safely use both medications. You should avoid the use of alcohol while being treated with ramelteon. Avoid driving or operating hazardous machinery until you know how the medication affects you, and do not exceed the dosage or frequency of use prescribed by your doctor. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Professional:

MONITOR: The concomitant use of doxepin and ramelteon may increase the systemic exposure to ramelteon. The mechanism has not been reported. After administration of doxepin 10 mg/day for 23 days and a single 8 mg dose of ramelteon, the overall exposure (AUC) to ramelteon increased by approximately 66% and the Cmax increased by 69%.

MANAGEMENT: Close monitoring of patients is recommended during coadministration.
