Consumer:

Using rucaparib together with warfarin may cause you to bleed more easily. You may need a dose adjustment based on your prothrombin time or International Normalized Ratio (INR). Call your doctor promptly if you have any unusual bleeding or bruising, vomiting, blood in your urine or stools, headache, dizziness, or weakness. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Professional:

GENERALLY AVOID: Coadministration with rucaparib may increase the plasma concentrations and the risk of adverse effects of drugs that are substrates of CYP450 2C9, such as warfarin. The proposed mechanism is decreased clearance due to rucaparib-mediated inhibition of CYP450 2C9. In a clinical study, rucaparib increased S-warfarin Cmax and AUC by 1.05- and 1.49-fold, respectively.

MANAGEMENT: Concomitant administration of rucaparib with warfarin should generally be avoided; however, if coadministration cannot be avoided caution and close monitoring are advised. The INR should be checked more frequently and warfarin dosage adjusted accordingly, particularly following initiation, change of dosage, or discontinuation of rucaparib therapy. The same precaution may be applicable during therapy with other coumarin anticoagulants, although clinical data are lacking. Patients should be advised to promptly report any signs of bleeding to their doctor, including pain, swelling, headache, dizziness, weakness, prolonged bleeding from cuts, increased menstrual flow, vaginal bleeding, nosebleeds, bleeding of gums from brushing, unusual bleeding or bruising, red or brown urine, or red or black stools.
