Short-acting formulations of octreotide may significantly reduce the blood levels of telotristat ethyl when both are given at the same time. If you are prescribed a short-acting octreotide with telotristat ethyl, you should administer the octreotide injection at least 30 minutes after taking telotristat ethyl to minimize the interaction. ADJUST DOSING INTERVAL: Concurrent administration with short-acting octreotide may significantly decrease the systemic exposure to both telotristat ethyl and its active metabolite, telotristat. The mechanism of interaction has not been established. When a single 500 mg dose of telotristat ethyl (twice the recommended dose) was administered with short-acting octreotide 200 mcg injected subcutaneously, mean telotristat ethyl peak plasma concentration (Cmax) and systemic exposure (AUC) decreased by 86% and 81%, respectively. The mean Cmax and AUC of telotristat also decreased by 79% and 68%, respectively. The in vitro inhibitory potency of telotristat towards tryptophan hydroxylase has been shown to be approximately 29 times higher than that of the parent drug.

MANAGEMENT: Short-acting octreotide should be administered at least 30 minutes after telotristat ethyl.
