Consumer:Terfenadine is no longer on the market in the US. Do not take terfenadine with any other medication.
Professional:GENERALLY AVOID: A single case report describes the occurrence of torsades de pointes due to the addition of terfenadine therapy to a patient stable on sotalol. The arrhythmia presented 7 days after terfenadine was started. The mechanism is unknown, but is thought to be related to cumulative effects of the drug combination on the QT interval. Other non-sedating antihistamines that can prolong the QT interval may interact similarly.
MANAGEMENT: Alternate antihistamine therapy, such as cetirizine, fexofenadine or loratadine is recommended.
