Consumer:Using ALPRAZolam together with nefazodone may increase the effects of ALPRAZolam. Professional:ADJUST DOSE: Nefazodone, when administered with alprazolam, has caused a two-fold increase its half-life, area under the plasma concentration-time curve, and peak plasma concentrations. The mechanism is inhibition of hepatic CYP450 3A4 metabolism by nefazodone.
MANAGEMENT: The manufacturer recommends a 50% reduction in the initial dosage of alprazolam. Patients receiving this combination should be monitored for excessive and prolonged sedation. Lorazepam, oxazepam, and temazepam undergo conjugative metabolism and may be suitable alternatives.
