Using bosentan together with simvastatin may decrease the effects of simvastatin. Contact your doctor if your condition changes or if your cholesterol increases. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

MONITOR: Coadministration with bosentan may decrease the plasma concentrations of simvastatin and other HMG-CoA reductase inhibitors that are metabolized by the CYP450 3A4 isoenzyme. The mechanism is accelerated clearance due to induction of the isoenzyme by bosentan. According to the manufacturer, bosentan decreased the plasma concentrations of simvastatin and its active beta-hydroxy acid metabolite by approximately 50%.

MANAGEMENT: The possibility of diminished HMG-CoA reductase inhibitory activity should be considered during concomitant use of atorvastatin, cerivastatin, lovastatin, simvastatin, or red yeast rice (which contains lovastatin) with bosentan. Cholesterol levels should be checked following initiation or discontinuation of bosentan therapy, and the dosage of HMG-CoA reductase inhibitor adjusted accordingly. Pravastatin and fluvastatin may be reasonable alternatives, since they are metabolized by other hepatic enzymes.
