Talk to your doctor before using boceprevir together with efavirenz. Combining these medications may decrease the blood levels of boceprevir, which may reduce its effectiveness in treating hepatitis C. Your doctor may be able to prescribe alternatives that do not interact. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

GENERALLY AVOID: Coadministration with efavirenz may decrease the plasma concentrations of boceprevir. The mechanism is efavirenz induction of CYP450 3A4, the isoenzyme responsible for the metabolic clearance of boceprevir. When boceprevir (800 mg three times daily for 6 days) was given in combination with efavirenz (600 mg once daily for 16 days), boceprevir peak plasma concentration (Cmax), systemic exposure (AUC) and trough plasma concentration (Cmin) decreased by 8%, 19% and 44%, respectively. In contrast, efavirenz Cmax increased by 11% and AUC increased by 20% in the presence of boceprevir. The significant reduction in trough concentrations of boceprevir may lead to virologic failure.

MANAGEMENT: Given the risk of reduced viral susceptibility and resistance development associated with subtherapeutic drug levels, concomitant use of boceprevir with efavirenz should generally be avoided.
