Consumer:Using tacrolimus together with verapamil may increase the effects of tacrolimus. Contact your doctor if you experience increased side effects such as fever, infection, diarrhea, tremor, headache, fatigue, or lethargy. If your doctor does prescribe these medications together, you may need a dose adjustment or special test to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.
Professional:MONITOR: In vitro data suggest that some calcium channel blockers inhibit the CYP450 3A4 hepatic metabolism and produce elevated blood levels of macrolide immunosuppressants. In healthy subjects, diltiazem 120 mg increased the maximum plasma concentration of sirolimus 1.4-fold and the AUC 1.6-fold. In contrast, limited information suggests that verapamil concentrations may also increase when coadministered with sirolimus.
MANAGEMENT: Careful monitoring of macrolide immunosuppressant and possibly calcium channel blocker blood concentrations is recommended if these drugs must be used together. Dose reductions of the immunosuppressant may be required.
