Consumer:

Cimetidine can increase the blood levels and effects of tamsulosin. This may cause blood pressure to fall excessively and heart rate to increase, especially when you rise from a sitting or lying position. The risk of other side effects such as dizziness, lightheadedness, fainting, headache, flushing, nasal congestion, heart palpitations, and priapism (prolonged and painful erection unrelated to sexual activity) may also increase. You may need a dose adjustment or more frequent monitoring by your doctor to safely use both medications. Let your doctor know if you develop these symptoms while using tamsulosin and they do not go away on their own or they become troublesome. Avoid driving or operating hazardous machinery until you know how the medication affects you, and use caution when getting up from a sitting or lying position. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Professional:

MONITOR: Coadministration with cimetidine may increase the plasma concentrations of tamsulosin. The proposed mechanism is cimetidine inhibition of tamsulosin metabolism via CYP450 3A4 and 2D6. In 10 healthy volunteers, administration of a single 0.4 mg dose of tamsulosin with cimetidine at the highest recommended dosage (400 mg every 6 hours for 6 days) resulted in a 26% decrease in tamsulosin clearance and a 44% increase in tamsulosin systemic exposure (AUC) compared to administration alone.

MANAGEMENT: Caution is advised if tamsulosin is used concomitantly with cimetidine, particularly at a dosage higher than 0.4 mg/day. The potential for increased risk of adverse effects such as postural hypotension, syncope, and priapism should be considered. Patients should be advised to avoid rising abruptly from a sitting or recumbent position and to notify their doctor if they experience dizziness, lightheadedness, syncope, orthostasis, or tachycardia. Patients should also avoid driving or operating hazardous machinery until they know how the medication affects them.
