Consumer:GuanFACINE may increase the concentrations of valproic acid, which may lead to an increased risk of side effects such as dizziness, drowsiness, confusion, and difficulty concentrating. Professional:MONITOR: Coadministration with guanfacine may increase plasma concentrations of valproic acid. The mechanism has not been delineated; however, it has been suggested that it may involve competitive inhibition of the glucuronidation metabolic pathway. Both valproic acid and guanfacine are metabolized by glucuronidation. Increased concentrations of valproic acid may lead to an increased risk of central nervous system (CNS) side effects. However, pharmacokinetic and clinical data are lacking.
MANAGEMENT: Caution is advised if valproic acid is used with guanfacine. Dosage adjustment as well as clinical and laboratory monitoring of valproic acid should be considered. Patients should be advised to notify their health care professional if they experience increased CNS effects, including dizziness, drowsiness, lightheadedness, confusion, or fainting.
