Coadministration with conivaptan does not appear to significantly affect the pharmacokinetics of warfarin, which undergoes major metabolism by CYP450 2C9 and minor metabolism by CYP450 3A4. In one study, administration of a single 25 mg dose of warfarin in combination with conivaptan (40 mg/day for 4 days) resulted in a 17% increase in the mean peak plasma concentration (Cmax) of the more active S(-) enantiomer of warfarin and a 14% increase in systemic exposure (AUC) compared to administration of warfarin alone. The corresponding prothrombin time and international normalized ratio values were not altered.
